Appointments are recommended for health care visits. Call 871-2400 to schedule. If you cannot keep an appointment, please notify the health center at least 2 hours in advance. The appointment time can then be offered to another student.
My healthcare provider at home ordered lab tests, but it is not convenient for me to go back home for the tests. Where can I get the lab tests done?
With a request from your health care provider, Crandall Health Center may be able to obtain your lab specimens (ex. blood, urine) and send them to St. James Mercy Health's laboratory for processing.
- Your health care provider must complete the following form, "Request for Laboratory Services at Crandall Health Center, Lab Work Obtained at CHC External Provider Form. Use of this required form ensures your lab results will be communicated directly to your health care provider. The form can be faxed to Crandall Health Center (607-871-2631). The form may also be mailed or hand delivered; it is required that the health center receive this form before your appointment is scheduled.
- You must make an appointment (607-871-2400) for the lab specimens to be obtained at Crandall Health Center. This service is available Monday through Thursday, 9am-2pm. There will be a venipuncture fee for blood draws, which can be billed to your AU student account. Fees for the actual laboratory tests are billed through St. James Mercy Health.
- Some laboratory tests require specific handling and must be done in a laboratory setting. For those needs, you may contact St. James mercy Health Laboratory at 607-324-8080 or Jones Memorial Hospital Laboratory at 585-593-4072 for more information.
Where do I go for medical care when Crandall Health Center is closed?
There are a variety of options for health care in the Alfred area during college breaks or over the summer. Remember, if you have a health care provider/family doctor in the area, you should consider calling their office first. Other options include:

-Jones Memorial Clinic, 28 Church Street, Alfred, NY. Call for appointment: 607.587.8143. Open Monday through Friday.
- Hornell Surgical Group, Same Day Clinic, 32 Genesee Street, Hornell, NY. Call for appointment: 604.324.1000. Open Monday through Friday.
-AU Rescue Squad operates when school is in session beginning at 6pm daily. Call 2108 for information.
Will Crandall Health Center give me an excuse if I miss class? The Health Center does not provide excuses for classes missed due to routine illnesses. Students are encouraged to communicate directly with instructors regarding missed class time and any necessary arrangements for making up missed work. With specific written permission from the student, Crandall Health Center will notify professors when it has been determined by Health Center staff that the student should not be attending class. There are no charges for office visits to the Center with the exception of Psychiatric Nurse Practitioner visits. Nominal charges are made for lab tests, injections, special treatments and medications. Students who require specialists or emergency care are referred to the general hospitals in neighboring Hornell or Wellsville. Students maintain the right to choose a health care provider or place of hospitalization and must assume all financial obligations for off-campus medical/surgical care.

How Do I Get A Copy of My Immunization Health
Record?
To obtain a copy of your immunization record, please click here to download the appropriate release form. Mail the completed release form to: Crandall Health Center at Alfred University, Attention: Immunization Records, 19 Park Street, Alfred, NY 14802. Enclose a check or money order for $2.00 to cover standard processing costs. Your immunization record will be mailed to you at the address indicated on the release form. Crandall Health Center agrees to send immunization records to you within 30 days of receiving your request.
Important Note:
Immunization records will be mailed directly to you; they will not be faxed and they will not be forwarded to other institutions. This is due, in part, to federal HIPPA laws. It will be your responsibility to forward these records to other schools, employers, etc. Be sure to keep a copy of your immunization record in a safe place for your future needs. Copies of health records are destroyed after 10 years of absence from Alfred University. If you are requesting copies of health records other than Immunization Records, please contact Crandall Health Center regarding that process.
